Ankle Pain Without Injury: Understanding the Causes and Treatment Options
Sudden ankle pain without injury can be a concerning symptom, but understanding the potential causes can help in seeking appropriate treatment. Common Causes and Symptoms
- Osteoarthritis: A wear-and-tear joint disease causing gradual cartilage degeneration. Symptoms include pain worsening with activity, stiffness (especially in the morning), swelling, and reduced range of motion.
- Rheumatoid Arthritis: An autoimmune condition causing symmetric joint inflammation. Symptoms include joint swelling, pain, stiffness (morning stiffness lasting more than an hour), and sometimes systemic symptoms like fatigue.
- Reactive Arthritis: An inflammatory arthritis triggered by infection elsewhere in the body. Symptoms can include joint pain and swelling, often accompanied by redness and warmth.
- Lupus (Systemic Lupus Erythematosus): An autoimmune disease causing joint inflammation, often accompanied by skin rash, fatigue, and organ involvement.
- Flat Feet (Pes Planus): Abnormal foot alignment leading to reduced arch height increases strain on ankle joints and tendons, causing pain, especially after walking or standing, sometimes contributing to tendonitis or arthritis.
- Achilles Tendonitis: Inflammation of the Achilles tendon at the back of the ankle, causing localized pain worsening with load-bearing or walking.
- Gout: A form of inflammatory arthritis due to uric acid crystal deposits. Presents with sudden onset of severe, intense pain, redness, swelling, and warmth around the joint.
- Bursitis: Inflammation of fluid-filled sacs around joints. Causes localized pain, swelling, and tenderness that worsen with movement.
- Tarsal Tunnel Syndrome: Compression of the tibial nerve in the ankle, causing burning, tingling, numbness, or shooting pain on the inside of the ankle and sole.
- Sinus Tarsi Syndrome: Irritation or inflammation of the sinus tarsi (a small cavity on the outside of the ankle) causes lateral ankle pain, tenderness, a feeling of instability, and difficulty bearing weight.
Treatment Options
Treatment options vary by cause but generally include:
- Arthritis (Osteoarthritis, Rheumatoid, Lupus, Reactive arthritis): Anti-inflammatory medications (NSAIDs), disease-modifying antirheumatic drugs (DMARDs) or biologics for RA and lupus, physical therapy, joint protection with supportive footwear or orthotics, sometimes corticosteroid injections, and lifestyle modifications like weight management and low-impact exercise.
- Achilles Tendonitis and Bursitis: Rest, ice application, physical therapy, heel lifts or cushioned footwear, and in severe cases, corticosteroid injections or surgery.
- Gout: Acute attacks: NSAIDs, colchicine, or corticosteroids; long-term: uric acid-lowering medications and dietary changes to reduce purine intake.
- Flat Feet and Related Pain: Orthotic devices to support the arch, proper footwear, and physical therapy to strengthen foot and ankle muscles.
- Tarsal Tunnel and Sinus Tarsi Syndromes: Rest, anti-inflammatory medications, physical therapy, and in difficult cases, nerve decompression surgery or corticosteroid injections.
